I am correct in thinking that in order to have my logo displayed on the PayPal order form I have to sign up for the reseller program and place your button on my site? If so, on how many pages must it be displayed? How will you confirm that it is displayed?

You can place it on a single page. As soon as you view it on the page you've posted it, you will be eligible to upload your logo in E-junkie Seller Admin > Profile.
