What happens if someone's payment through Paypal is 'uncleared'? Do they still get the digital file, or do they not get it until their payment clears?

Pending orders are not processed by E-junkie unless you have specifically instructed E-junkie to process pending orders as normal orders (the setting is in E-junkie Seller Admin > Preferences).
