I would like to cancel my account. Your service would be great for a normal website, but i need to have the ability to attach customized forms to my orders.

You can cancel the subscription from Admin panel itself.

Just so you know, you can provide the custom form on the "thank you" page.

In the custom form, is there a variable I can use for the buyers address and zip code? I can see city, state and country - but no address or zip code.

Also FYI on the customization help page you have the following mistake:

[%city%] (Buyer's City[if applicable])
[%state%] (Buyer's City[if applicable]) <------ should read "(Buyer's State[if applicable])"
