I currently sell all e-products.

But now I would like to add an actual physical product via drop shipping. If I adjust the cart settings in my account, will this apply to my e-products too (I don't want it to)? Do I have to set up a whole new account for tangible items?

You can add tangible products in the same account with digital goods. When you set up Shipping calculation for your account, that will only apply to products which have their Shipping/Buyer's Address setting enabled:
