To issue a refund, do I go through e-junkie, or Paypal?

You will issue a refund from the payment processor's website (PayPal/Google Checkout etc.)

How do we take that customer off the list for future download links for software upgrades?

at the admin screen, under manage buyers, click block buyers / transactions. from there you can enter the name or email of the person you want to block, but it wont stop them from purchasing, only from downloading your product. you will have to refund the money later with your payment processor.

For PayPal and Google Checkout payments, when a payment is refunded or reversed, the buyer is automatically taken off the mailing list.
