Flat rate shipping - three different rates

I'm usually pretty good with this stuff and have looked through similar posts but I'm just confused.

I only sell one tangible product on my site and it's doing pretty well so I want to expand to offer an affiliate program, that's why I want to use e-junkie. Right now everything works fine in PayPal but I realize I have to configure the shipping in e-junkie if I want to use the e-junkie affiliate service.

I currently offer flat rate shipping to destinations as follows:

USA - Free shipping
Canada - $5.00

$ 9.00 for Australia, Belgium, Denmark, Finland, France, Germany, Ireland, Israel, Italy, Japan, NewZeland, The Netherlands, Norway, Spain, Switzerland, and the UK.

I also charge sales tax in CA - can I use the "Buy Now" buttons, or do I need to use the "Add to cart" buttons for the sales tax to be properly calculated?

Any help would be GREATLY appreciated!

Taking the tax question first, you will need to use E-junkie Cart buttons in order to use our tax calculation feature:

Now, for shipping, make sure you enable Shipping/Buyer's Address in the settings of each shippable product, and follow these steps to set up shipping calculation:

Go to Seller Admin > Cart Shipping Settings and set up the following:
# Shipping Origin: select United States and enter your zipcode;
# Shipping Destinations: if you will only ship to certain countries, select them in the list;
# Leave all other settings blank or 0.00;
# Click Next to save settings and proceed to the Shipping Rules screen.

In the Existing Shipping Rules menu, select and Delete any Rules you may have saved there.

USA:
# Shipping Rate Calculator: Flat
# Shipping Amount: 0.00
# Country: United States;
# Click Submit to save the Rule.

Canada:
# Shipping Rate Calculator: Flat
# Shipping Amount: 5.00
# Country: Select a country;
# Click Submit to save the Rule.

Others:
# Shipping Rate Calculator: Flat
# Shipping Amount: 9.00
# Country: All Countries (i.e. the rest of your Shipping Destination countries);
# Click Submit to save the Rule.

Click Back to Admin, and you're done!

Thanks so much for the help! It works perfect for the USA and calculates California sales tax perfectly... however - it doesn't work at all for Canada and the rest of my countries. Don't know what in the heck I could be doing wrong? I thought I followed your instructions to the letter...

I found out that it seems to work OK if the correct postal code is used and then they click "Update Cart." If they don't put in the postal code then it doesn't charge them any shipping. Will the system allow them to actually checkout without putting in the postal code, or will it prompt them for a postal code if they try to checkout without entering one?

If buyers do not enter a postal code in the cart when prompted they won't be able to proceed to a checkout page. An error message will appear prompting them to enter their postal code/zip code.
