How can I add "special handling" instructions to the checkout process?

For example, I have some customers who would like to request for special instructions for their packages such as "leave at the door", or "leave at the leasing office".

We don't really have a way for buyers to enter a general comment about their order in the cart before checkout, but some payment processors such as PayPal provide a Special Instructions or Note to Seller field during checkout on their site.

You might also consider customizing your E-junkie-generated thank-you pages to add a contact form buyers could use to send a note. If you're not selling downloads, you could just put a contact form on a thank-you page on your own site and redirect all buyers there (in Seller Admin > Account Preferences > Common Thank-you Page URL).
