I have a download product (ebook/music) zipped and need to include a small musical instrument (recorder) from a music store (unknown thus far). Affiliate sales are a problem in North Carolina. Comission Junction will not allow NC residents to participate... and don't know yet if Google Affiliate Network will.

I believe I need to include a recorder in the sale. I do not want to sale/mail recorders, so there must be someone willing to do it, and I don't care if I get a commission. Obviously it would be nice.

Can I do this with one sale/checkout using only ejunkie?
How can I handle returns?

As long as setup the product to require the buyer to include their shipping address the information that is forwarded will include everything the third-party needs to ship the recorder.
