For example: For the entire order, they need to give me special instructions like: Please seat us with these people. or This delivery needs to be left with a callback for special spellings, etc...

This isn't really an ideal solution if you are trying to give away a separate product for free, however. Entering an option will not actually add another product to the cart but if you are dealing in tangible goods and not tracking inventory having an option that lets you know what else to include with an item when it ships could work.

For example, I'd like to offer a buy 1, get 1 of something else free deal, and have customer be able to specify what they want for free in the box.
